FlashLoopAdapter
Oct 1, 2026
FlashLoopAdapter’s open()/close() authorization trusted any caller whose isModuleEnabled() response returned true, allowing an attacker-controlled fake Safe to pass the check. ) as arbitrary swapCalldata.
Because FlashLoopAdapter was already an enabled module on those Safes, they accepted the calls and transferred or withdrew collateral to the attacker.
How the exploit worked
View reproducible PoCa Safe module authenticates its caller by asking that caller whether the module is enabled (self-attestation) and then makes an unconstrained call to a caller-supplied router, letting a fake Safe drive the adapter into draining real Safes that enabled it.
Lost: 114.096151 WETH (~$305k, exactly reproduced) on Ethereum; two Safes that enabled the module as a victim were emptied - Safe #1 lost 1306.48 weETH of Aave col…
